Sivelestat, also known as ONO 5046, is a competitive inhibitor of human neutrophil elastase with an IC50 of 0.044 µM. Sivelistat also inhibits leukocyte elastase obtained from rabbit, rat, hamster and mouse but does not inhibit trypsin, thrombin, plasmin, plasma kallikrein, pancreas kallikrein, chymotrypsin or cathepsin G even at 100µM. Sivelistat is used in the treatment of acute respiratory failure and also improves neuropathic pain.
