Qualitative detection of SARS-CoV-2 antibodies (IgM and IgG) as an indicator for a previous infection
The qualitative detection of SARS-CoV-2 antibodies (IgM and IgG) is mainly intended as an aid to determine a previous natural exposure to the Corona Virus. The tests do not allow differentiation between antibody induced by infection or antibodies induced by vaccination. The detected antibodies can be directed against one viral antigen such as spike protein, nucleocapsid or a mixture of antigens.
For detection of COVID-19 RT-PCR tests remain the gold standard for diagnosis of acute infections with the novel SARS CoV-2 virus.

How can the test for neutralizing antibodies help to determin the status of immunity after vaccination?
Not all antibodies are created equal. After Infection with the SARS-CoV-2 the body reacts by producing a multitude of antibodies against different viral proteins (learn more about SARS-CoV-2 Antibodies). But only neutralizing antibodies bind to proteins on the surface of the corona virus that either sterically prevent the pathogen from binding to the host cell or prevent the conformational change of proteins that is necessary for entry into the host cell. In corona viruses, neutralizing antibodies are often directed against the 2019-nCoV spike protein receptor binding doamain (RBD). Their presence can be interpreted as evidence of immunity to COVID-19 (Khoury et al, Nature Medicine 2021). Antibody tests that do not specifically detect neutralizing antibodies have less predictive value about the status of the immunity, but prove a previous infection with SARS-CoV-2.
How long does it take for antibodies against COVID-19 to form?
Antibodies against SARS-CoV-2 are formed approximately from the 7th day after infection (IgM and IgA) and remain detectable in the blood for many weeks (IgG). Antibody tests are suitable for the confirmation of an infection in the late stage and for the assessment of the immune status after infection.
