FGF23: Novel Risk Marker of Heart Failure
FGF23 (fibroblast growth factor 23), a known marker of phosphate metabolism, has acquired independent importance in the cardiovascular system (Hu et al. 2013). Recent studies indicate that FGF23 is also expressed in the heart (Itoh et al., 2016). Elevated FGF23 levels are associated with cardiovascular mortality, higher risk of myocardial infarction, stroke, and heart failure. In addition, there is growing evidence that FGF23 levels may predict response to therapies to reduce the risk of heart failure (Brandenburg et al., 2014; Almahmoud et al., 2018). Further clinical studies are needed, but FGF23 is one of the few promising biomarkers that have the potential to determine the effect of FGF23-specific therapeutic interventions on clinically relevant endpoints (Vervloet et al., 2019).

big ET for the diagnosis of Heart Failure with preserved Ejection Fraction (HFpEF)
Renal dysfunction may limit the clinical application of NT-proBNP in the diagnosis of heart failure. Big ENDOTHELIN (big ET) is an excellent biomarker candidate for the diagnosis of HFpEF in CKD, independent of GFR (Gergei et al., 2018). NT-proBNP is strongly associated with GFR and shows an exponential negative correlation with GFR decline, whereas Big ET-1 is much less affected. A single cut-off point is sufficient in the diagnosis of HFpEF in CKD.

- ENOS/NO signaling pathway: As a signal molecule, nitric oxide plays an important role in regulating blood pressure, maintaining vascular tension and regulating immune system in physiological activities. - MTOR signaling pathway: mTOR kinase is an essential fator in regulating the homeostasis and growth of cardiomyocytes during development and postnatal period.
- NOTCH signaling pathway: Notch signaling can be partly responsible for a variety of physiological processes, such as angiogenesis, myogenesis, neurogenesis and hematopoiesis, and determine the growth, differentiation and survival of various cell types in different tissues.

Common Cardiovascular Diseases
- Atherosclerosis is the narrowing of arteries caused by the buildup of fatty plaques, typically containing LDL cholesterol, smooth muscle cells, fibrous tissue, and sometimes calcium. Artherosclerosis is the main cause of coronary heart disease, cerebral infarction, and peripheral vascular disease.
- Coronary Artery Disease (CAD) develops when the coronary arteries become damaged or diseased. CAD then causes by a buildup of plaque in coronary arteries.
- Heart attack (myocardial infarction) occurs when the flow of blood to the heart is blocked. It most often causes by a build-up of fat, cholesterol and other substances, which form a plaque in the coronary arteries.
- Stroke is usually divided into ischemic stroke and hemorrhagic stroke. An ischemic stroke occurs when a blood vessel that feeds the brain gets blocked, usually from a blood clot. A hemorrhagic stroke occurs when a blood vessel within the brain bursts. This is most often caused by uncontrolled hypertension (high blood pressure).
